Billing Information

For most PIs/lab. managers/users just fill in the dark blue regions of the form.  Percentage will be assumed to be 100%. 

PIs/lab. managers/users who wish to spread charges over multiple accounts should also fill in lines 2 etc.  Either a fraction of the charges can be apportioned (using percentage specified) to the specified grants or alternatively a user can have multiple logon accounts, and have activity in each logon billed to separate accounts.  Please contact Michael for clarification.

 New users may choose a logon name   (typically this name is used in line 1 below, see note 1)
 Choose a BookingID   4 digit ID code for booking/canceling time on the calendars
